Time Off with Pay

  • Holidays - Eligibility: Full & Part-Time Employees scheduled for more than 20 hours per week
    • Employer paid
    • Seven full holidays per year
    • Two half holidays per year
  • Vacation Accruals - Eligibility: Full & Part-Time Employees scheduled for more than 20 hours per week
    —Employer paid
    • Full-Time  Exempt and Non-Exempt:
      —104 hours - During 1st two years
      —144 hours – 2–4 years
      —184 hours - After four years
    • Part-Time scheduled for more than 20 hours per week
      —2.60 hours per pay period - During 1st two years
      —3.00 hours per pay period -2-4  years
      —4.00 hours per pay period – After four years
  • Sick Leave - Eligibility: Full-Time Employees
    • Employer paid
    • After 180 days of active employment - Example: Five days are available per year for the first three years of employment.

Medical Insurance (eligible upon employment)

Eligibility: Full & Part-Time Employees scheduled for more than 20 hours per week

Two medical plans to choose from:

  • PPO
    • 80% inpatient in-network - Deductibles apply
    • 80% office visits in-network - Deductibles apply
    • Prescription Drug - 31 day supply
  • HMO
    • 100% inpatient in-network - Deductibles apply
    • Office visit co-pay - No deductible
    • Prescription Drug - 31 day supply

Dental Insurance (eligible upon employment)

Eligibility: Full & Part-Time Employees scheduled for more than 20 hours per week  

  • Employee & Employer paid
  • Calendar year maximum - $1,500 for an individual
  • Ortho lifetime maximum - $1,500 per child
  • Diagnostic and Preventive - 100%
  • Maintenance and Simple Restorative - 80%*
  • Complex Restorative - 80%*
  • Orthodontic care - 60%* - dependent children only
    *Based on usual and customary changes. Subject to deductible.

Group Life Insurance

  • Basic Life & Accidental Death and Dismemberment Insurance - Eligibility: Full-Time Employees
    • Employer paid
    • 25 x monthly salary
  • Additional Life & Accidental Death and Dismemberment Insurance - Eligibility: Full-Time Employees
    • Employee paid
    • max 5 x annual salary up to $500,000 life insurance
    • max $250,000 Accidental Death and Dismemberment

Disability Insurance

  • Salary Continuance - Eligibility: Full-Time Employees after 180 days of active employment
    • Employer paid
    • 55% of pay
  • Long-Term Disability - Eligibility: Full-Time Employees after six months waiting period
    • Employee Paid - Salary adjusted to equal premium costs
    • 55% of salary after six months waiting period
    • Maximum $7,500 per month
    • Eligible after 6 months of active employment

Retirement Program

401(k) - Eligibility: Full-Time, Part-Time and Temporary Employees

  • Employee & Employer paid
  • Administered by J. P. Morgan
  • Match of $.50 for every dollar you contribute up to 6% of  your compensation
  • 3% annual contribution if employee worked 1,000 hours in the calendar year
  • Vesting:
    • 0% - less than 2 years
    • 25% - 2 years
    • 50% - 3 years
    • 75% - 4 years
    • 100% - 5 years
